ABSTRACT

Emergências hospitalares são portas de entrada para pacientes graves com potencial risco de morte. Em um hospital referência em trauma, a função do fisioterapeuta dentro da equipe multiprofissional ainda tem necessidade de ser bem estabelecida. Objetivo: Discorrer sobre a atuação do fisioterapeuta no serviço de emergência de um hospital de pronto socorro referência em trauma e apresentar o perfil do paciente atendido. Método: Trata-se de uma pesquisa observacional descritiva, de caráter transversal retrospectiva a partir da análise dos atendimentos fisioterapêuticos registrados em prontuários de pacientes hospitalizados. Resultados: As causas mais frequentes de internação dos pacientes atendidos pela equipe de fisioterapia foram quedas (51,7%), acidentes de trânsito (14,9%), cardiovasculares (9,2%), agressões (9,1%), outros (8,3%) e respiratórias (6,8%). Quedas da própria altura com fratura de fêmur associadas representaram 26,4% da amostra. Nestes pacientes as condutas mais utilizadas pelos fisioterapeutas foram orientações, exercícios respiratórios, exercícios no leito e ajustes de posicionamento. O fisioterapeuta teve atuação com pacientes com pneumotórax, hemotórax ou hemopneumotórax submetidos à drenagem torácica, por meio de exercícios respiratórios expansivos, saída do leito e deambulação precoce. Observou-se também atuação com os pacientes em ventilação mecânica invasiva e não invasiva na sala vermelha. Conclusão: O fisioterapeuta, fazendo parte da equipe multiprofissional, apresenta importante atuação nas salas de emergência de um hospital de trauma, tal como o atendimento a pacientes submetidos a ventilação mecânica invasiva e não invasiva, uso de técnicas de fisioterapia respiratória para expansão pulmonar e remoção de secreções bem como a utilização de técnicas de cinesioterapia para manutenção e ganho de força muscular e funcionalidade.


Hospital emergencies are gateways to critically ill patients with potential risk of death. In a trauma referral hospital, the physical therapists role within the multiprofessional team must still be well established. Objective: To discuss the role of the physical therapist in the emergency department of a reference trauma hospital and to present the profile of the patients admitted to hospitalization. Method: This is a descriptive, observational, cross-sectional study that analyzes physiotherapy and records of hospitalized patients. Results: The most frequent causes of hospitalization of patients treated by the physiotherapy team were falls (51.7%), traffic accidents (14.9%), cardiovascular diseases (9.2%), assaults (9.1%), and respiratory (6.8%). Falls from standing height with associated femur fractures represented 26.4% of the records. With these patients, the procedures most used by physical therapists were orientations, breathing exercises, bedside exercises, and positioning adjustments. With lung-expanding breathing exercises, bedside exercises, and early walking, the physical therapist treated patients with pneumothorax, hemothorax, or hemopneumothorax who underwent thoracic drainage. Physical therapists also treated patients under invasive and non-invasive mechanical ventilation in the red ward. Conclusion: The physical therapist, as part of the multiprofessional team, has an essential role in the emergency rooms of a trauma hospital, such as care for patients undergoing invasive and non-invasive mechanical ventilation, use of respiratory physiotherapy techniques for lung expansion and removal of secretions, as well as the use of kinesiotherapy techniques to maintain and gain muscle strength and functionality.

ABSTRACT

Introducción: El trauma continúa siendo una importante causa de mortalidad en pacientes jóvenes en todo el mundo. Representa un desafío para los sistemas sanitarios, por ser un problema de salud pública. Su manejo en Chile, tradicionalmente, ha sido realizado por cirujanos generales, en servicios de urgencia, ya que no existe la cirugía de trauma como especialidad en nuestro país. Tenemos algunos cirujanos de trauma formados en otros países, pero en un pequeño número. El objetivo de esta revisión es saber cómo funciona, actualmente, el manejo del trauma en nuestro país y qué necesitamos para hacerlo adecuadamente, acercándonos a los estándares de países más desarrollados. Para ello realizamos una breve encuesta y una revisión de la literatura. Conclusión: si bien contamos con más conocimientos y recursos que en décadas pasadas, necesitamos políticas públicas que permitan tener sistemas de trauma, registro y centros de trauma.


Introduction: Trauma continues to be an important cause of mortality among young patients across the globe. It is a challenge for health systems, as it is a Public Health issue. Its management has traditionally been undertaken by general surgeons in Chile, in emergency romos. Trauma surgery does not existe as a specialty in our country. We have some trauma surgeons trained abroad, but in a small number. The aim of this article is to learn how Trauma Surgery works in our country, and what is needed to perform better; bringing it to the standard of highly developed countries. To achieve this we have conducted a brief poll and a narrative review of literature. Conclusion: although our knowledge and resources have improved in comparison to decades ago, we need public policies that allow for trauma systems, registries and trauma centers.

O trauma é responsável por significativos impactos na sociedade. De acordo com dados da Organização Mundial de Saúde (OMS), mais de nove pessoas morrem por minuto, vítimas de trauma. Entre os principais tipos de trauma, o torácico representa na atualidade cerca de 25% dos mortos em politraumatizados, constituindo um problema complexo, tendo em vista os elevados índices de mortalidade e sequelas incapacitantes permanentes. Objetivo: Descrever o perfil epidemiológico dos pacientes atendidos em um hospital de urgências da região centro-oeste, vítimas de trauma torácico. Método: Estudo quantitativo, de caráter transversal e retrospectivo, realizado a partir de coleta de dados efetuada em prontuário eletrônico no período de março a maio de 2022. Resultados: Identificou-se 73 pacientes vítimas de trauma torácico, com maior acometimento de pessoas do sexo masculino, com idade entre 26 e 35 anos. Como causa mais frequente, se destacaram os acidentes motociclísticos, resultando principalmente em lesões do tipo hemopneumotórax. Conclusão: o perfil epidemiológico dos pacientes vítimas de trauma torácico foi representado com maior frequência pelo sexo masculino, com idade entre 26 a 35 anos, causados predominantemente por acidentes motociclísticos, resultando na maioria das vezes em lesões do tipo hemopneumotórax


Trauma is responsible for significant impacts on society. According to data from the World Health Organization (WHO), more than nine people die per minute victims of trauma. Among the main types of trauma, thoracic trauma currently represents about 25% of polytrauma deaths, constituting a complex problem, in view of the high rates of mortality and sequelae permanent disabling. Objective: To describe the epidemiological profile of patients treated at an emergency hospital in the Midwest region, victims of thoracic trauma. Method: Quantitative, cross-sectional and retrospective study carried out from data collection of electronic medical records in the period from March to May of 2022. Results: We identified 73 patients who were victims of chest trauma with higher affecting males aged between 26 and 35 years. As the most frequent cause motorcycle accidents stood out, resulting mainly in lesions of the hemopneumothorax type. Conclusion: the profile epidemiology of patients victims of thoracic trauma was represented with greater frequency by males, aged between 26 and 35 years, caused predominantly by motorcycle accidents, often resulting in hemopneumothorax lesions

ABSTRACT Objective: To assess the impact of the COVID-19 pandemic on the epidemiology and clinical outcomes of open fractures considering the periods before and during the pandemic. Methods: An observational and retrospective study, which included patients aged over 18 years, admitted to the Orthopedics and Traumatology Ward of Hospital São Paulo, of the Federal University of São Paulo (UNIFESP). Data was collected in two moments: pre-pandemic (March 1, 2019, to February 29, 2020) and during the pandemic (March 1, 2020, to February 28, 2021). Results: In total, 183 patients were evaluated with a mean age of 36 years ± 14 years. In the pre-pandemic period, 94 patients underwent surgery, 81 men (85.37%) and 13 women (14.2%), with a mean age of 36 ± 3 years. During the pandemic period, 89 patients were subjected to surgery, 77 men (86.6%) and 12 women (13.4%), with a mean age of 38 ± 3 years. Conclusion: During the pandemic, open fractures were still more common in men. Regarding hospital indicators, the prevalence of infections in the surgical wound and the length of stay of patients with open fractures increased, however, with little significance. Fractures classified as Gustilo IIIA were the most common, while the most common according to the AO classification were 33, 34, 42, 43, 2R3, and 2R3 + 2U2. The frequency of run overs during the pandemic decreased. However, firearm projectile injuries and falls and occupational injuries increased. Level of Evidence III, Retrospective Comparative Study.


RESUMO Objetivo: Avaliar o impacto da COVID-19 na epidemiologia e nos desfechos clínicos das fraturas expostas considerando os períodos antes e durante a pandemia. Métodos: Estudo observacional e retrospectivo que incluiu pacientes maiores de 18 anos, internados na Enfermaria de Ortopedia e Traumatologia do Hospital São Paulo, da Universidade Federal de São Paulo. Os dados foram coletados em dois momentos - antes (março de 2019 a fevereiro de 2020) e durante a pandemia (março de 2020 a fevereiro de 2021) -, por meio da análise de prontuários eletrônicos de todos os registros hospitalares dessa instituição. Resultados: No total, foram avaliados 183 pacientes com média de idade de 36 ± 14 anos. No período pré-pandêmico, foram operados 94 pacientes, sendo 81 homens (85,37%) e 13 mulheres (14,2%), com média de idade de 36 ± 3 anos. Já ao longo do período pandêmico, foram operados 89 pacientes, sendo 77 homens (86,6%) e 12 mulheres (13,4%), com média de idade de 38 ± 3 anos. Conclusão: Durante a pandemia, a ocorrência de fraturas expostas se manteve com maior frequência em indivíduos do sexo masculino. Quanto aos indicadores hospitalares, houve aumento da prevalência de infecções na ferida operatória, assim como do tempo de internação dos pacientes, todavia, com pouca significância. Foram mais frequentes as fraturas classificadas como Gustilo IIIA e, pela classificação AO, as fraturas 33, 34, 42, 43, 2R3 e 2R3 + 2U2. Notamos redução da frequência de atropelamentos durante a pandemia e aumento dos casos de ferimentos por projétil de arma de fogo, quedas e acidentes ocupacionais. Nível de Evidência III, Estudo Retrospectivo Comparativo.

ABSTRACT Objective: to assess the epidemiological profile of trauma patients from fall from the same level (FSL) and fall from an elevated level (FEL) during the COVID-19 pandemic, and to compare it with data from different levels of restriction (flags) and data prior to the pandemic. Method: a cross-sectional study with a probability sample of the medical records of patients aged 18 years or older admitted to the emergency room due to falls, from June 2020 to May 2021. Epidemiological data, such as sex, age and injuries were analyzed, as well the current level of restriction. The three restriction periods were compared between then and the proportion of admissions due to falls was compared with the period from December 2016 to February 2018. Results: a total of 296 admissions were evaluated, 69.9% were victims of FSL and 30.1% of FEL. The mean age was 57.6 years, and 45.6% were over 60 years old. Admissions among men predominated, and 40.2% of patients required hospitalization. During the red flag period, there were proportionally more injuries to the head and neck (p=0.016), injuries to extremities (p=0.015) and neurological trauma (p<0.001). An average of 6.1, 6.3 and 5.2 admissions per day was obtained during the yellow, orange and red flag, respectively. There was a relative increase in falls when compared to the pre-pandemic period. Conclusions: there was an absolute reduction in admissions of victims of falls in midst of the most restrictive period during the pandemic. However, when compared to pre-pandemic data, there was a relative increase in falls.


RESUMO Objetivo: avaliar o perfil epidemiológico do trauma por quedas de mesmo nível (QMN) e quedas de nível elevado (QNE) durante a pandemia da COVID-19, realizar a comparação dos dados entre os níveis de restrição (bandeiras) e comparar com dados prévios à pandemia. Método: estudo transversal com amostragem probabilística de prontuários de pacientes com 18 anos ou mais admitidos na sala de emergência devido a quedas de junho de 2020 a maio de 2021. Foram avaliados dados epidemiológicos, como sexo, idade e lesões resultantes, além da bandeira vigente. Os três períodos de restrição foram comparados entre si e a proporção de atendimentos por quedas foi comparada com o período de dezembro de 2016 a fevereiro de 2018. Resultados: avaliou-se 296 atendimentos, sendo 69,9% vítimas de QMN e 30,1% de QNE. A média de idade foi 57,6 anos, sendo que 45,6% apresentavam idade superior a 60 anos. Sexo masculino predominou e 40,2% dos pacientes necessitaram internamento hospitalar. Durante a bandeira vermelha proporcionalmente ocorreram mais lesões em cabeça e pescoço (p=0,016), trauma em extremidades (p=0,015) e neurológico (p<0,001). Obteve-se uma média de 6,1, 6,3 e 5,2 atendimentos/dia durante a bandeira amarela, laranja e vermelha respectivamente. Ocorreu um aumento significativo da ocorrência de quedas quando comparado ao período prévio à pandemia. Conclusões: durante o período pandêmico, verificou-se uma redução absoluta de atendimentos de vítimas de quedas na bandeira mais restritiva. Porém, quando comparado ao período pré-pandêmico, verifica-se um aumento significativo das quedas.

ABSTRACT Objectives: to evaluate the profile of emergency care of trauma patients at Hospital Universitário Evangélico Mackenzie (HUEM) during the period of restrictive measures due to COVID-19 (03/13/2021 to 04/05/2021), and compare to the same period at the beginning of the pandemic, in 2020, and before the pandemic, in 2019. Methods: quantitative and descriptive observational cross-sectional study. The final sample of 8,338 was analyzed in terms of date, gender, age and service responsible for providing care; the traumas were analyzed according to the etiology and conduct of the treatment and outcome. Results: there was a percentage increase in non-traumatic emergency care during the pandemic, and the medical clinic held a third of admissions in 2021. There was a reduction in trauma care, since in 2019 traumas were responsible for 44.9% of admissions and by 23.5% in 2021. There was a significant difference in the proportion between the attendance of men and women, and the percentage of men victims of trauma was higher than in the pre-pandemic periods. There was a reduction in absolute numbers, with statistical significance, in traffic accidents, falls from the same level, burns, general blunt trauma and sports and leisure trauma. The proportion of conservative treatments with hospital discharge reduced. There was a significant difference in the number of deaths, decreasing in 2020 but increasing in 2021. Conclusion: there was a reduction in trauma care during the pandemic, but the profile remained the adult male victim of a traffic accident. More severe traumas were admitted, resulting in an increase in surgical treatment, hospitalizations and deaths.


RESUMO Objetivo: avaliar o perfil de atendimento emergencial dos pacientes vítimas de trauma do Hospital Universitário Evangélico Mackenzie (HUEM) durante o período de medidas restritivas devido à COVID-19 (13/03/2021 a 05/04/2021) e comparar ao mesmo período no início da pandemia, em 2020, e antes da pandemia, em 2019. Métodos: estudo transversal observacional quantitativo e descritivo. A amostra final de 8.338 foi analisada quanto a data, sexo, idade e serviço responsável pelo atendimento; os traumas foram analisados conforme a etiologia e a conduta do tratamento e desfecho. Resultados: houve aumento percentual no atendimento a urgências não traumáticas durante a pandemia, e a clínica médica deteve um terço das admissões em 2021. Ocorreu redução nos atendimentos por trauma, visto que em 2019 os traumas foram responsáveis por 44,9% das admissões e por 23,5% em 2021. Houve diferença significativa na proporção entre os atendimentos de homens e mulheres, sendo que o percentual de homens vítimas de traumas foi maior do que os períodos pré pandêmicos. Observou-se redução em números absolutos, com significância estatística, nos eventos de trânsito, queda de mesmo nível, queimaduras, traumas contusos gerais e traumas esportivos e de lazer. A proporção de tratamentos conservadores com alta hospitalar reduziu. Houve diferença significativa na quantidade de óbitos, reduzindo em 2020, mas aumentando em 2021. Conclusão: houve redução do atendimento no trauma durante a pandemia, mas o perfil permaneceu sendo o homem adulto vítima de eventos de trânsito. Traumas de maior gravidade foram admitidos, resultando no aumento de tratamento cirúrgico, internamentos e óbitos

ABSTRACT Introduction: trauma is the leading cause of death for the age group from 1 to 49 years in Brazil. Non-Operative Management (NOM) is the gold standard in trauma centers and does not affect mortality in comparison to operative treatment. Methods: medical records were reviewed for 114 patients with blunt liver trauma treated at Hospital das Clínicas of the Federal University of Uberlândia (HC-UFU) from November 2015 to November 2020. Results: the most prevalent gender was masculine (74.5%). The most prevalent age group was 20 to 49 years (65.7%). The majority of admitted patients (60.5%) had an Injury Severity Score (ISS) of more than 15. On hospital admission, 30.7% had HR above 100 bpm and 30.70% had SBP below 100mmHg. NOM was implemented in 77.2% of patients, the failure rate was 11.36% and the specific failure rate, excluding complications of associated injuries that resulted in surgery, was 1.75%. One third of deaths were due to severe traumatic brain injury. Conclusion: the failure rate of NOM in this study is similar to the literature reports for liver trauma. The failure rate, excluding complications of associated injuries, is considered low. The recognition of the epidemiological profile of patients admitted at HC-UFU allows multidisciplinary and integrated care with specialized training, as well as the development of institutional protocols, aiming to reduce morbidity and mortality related to hepatic trauma.


RESUMO Introdução: o trauma é a principal causa de morte na faixa etária de 1 a 49 anos no Brasil. O Tratamento não operatório (TNO) é padrão ouro nos centros de trauma e não altera a mortalidade quando comparado ao tratamento operatório. Métodos: foram avaliados 114 prontuários médicos de pacientes vítimas de trauma hepático contuso atendidos no Hospital das Clínicas da Universidade Federal de Uberlândia (HC-UFU) no período de novembro de 2015 a novembro de 2020. Resultados: os homens foram o sexo mais acometido (74,5%) e a faixa etária mais prevalente foi de 20 a 49 anos (65,7%). 60,5% dos pacientes admitidos apresentavam um Injury Severity Score (ISS) maior ou igual a 16. Na admissão, 30,7% apresentaram FC acima de 100bpm e 30,70% PAS abaixo de 100mmHg. O TNO foi instituído em 77,2% dos pacientes, o índice de falha foi de 11,36% e o índice de falha específica, excluindo as cirurgias decorrentes por complicação de lesões associadas, foi de 1,75%. 33,33% dos óbitos foram decorrentes de traumatismo cranioencefálico grave. Conclusão: o índice de TNO instituído no HC-UFU é próximo ao estabelecido na literatura para traumas hepáticos. O índice de falha, quando excluídas às complicações por lesões associadas, é considerado baixo. O reconhecimento do perfil epidemiológico dos pacientes admitidos no HC-UFU permite o treinamento especializado integrativo e multiprofissional das equipes assistenciais e o desenvolvimento de protocolos institucionais, visando reduzir a morbimortalidade relacionada ao trauma hepático.

Introducción. El objetivo de este estudio fue evaluar el impacto sobre la mortalidad según el perfil de ingreso a un centro de trauma del suroccidente colombiano, como método para entender las dinámicas de atención del paciente con trauma. Métodos. Se realizó un subanálisis del registro de la Sociedad Panamericana de Trauma asociado a un centro de trauma en el suroccidente colombiano. Se analizaron los pacientes atendidos entre los años 2012 y 2021. Se compararon los pacientes con condición de ingreso directo y aquellos que ingresaron remitidos. Se hicieron análisis de poblaciones de interés como pacientes con trauma severo (ISS > 15) y pacientes con/sin trauma craneoencefálico. Se evaluó el impacto de los pacientes remitidos y su condición al ingreso sobre la mortalidad. Resultados. Se incluyeron 10.814 pacientes. La proporción de pacientes remitidos fue del 54,7 %. Los pacientes que ingresaron remitidos presentaron diferencias respecto a la severidad del trauma y compromiso fisiológico al ingreso comparado con los pacientes con ingreso directo. Los pacientes remitidos tienen mayor riesgo de mortalidad (RR: 2,81; IC95% 2,44-3,22); sin embargo, es el estado fisiológico al ingreso lo que impacta en la mortalidad. Conclusión. Los pacientes remitidos de otras instituciones tienen un mayor riesgo de mortalidad, siendo una inequidad en salud que invita a la articulación de actores institucionales en la atención de trauma. Un centro de trauma debe relacionarse con las instituciones asociadas para crear un sistema de trauma que optimice la atención de los pacientes y la oportunidad


Introduction. This study aims to evaluate the impact on mortality by admission profile to a trauma center in Southwest Colombia between direct and referred patients, as a method to understand the dynamics of trauma care.Methods. A sub-analysis of the Panamerican Trauma Society registry associated with a trauma center in Southwest Colombia was performed. Patients attended between 2012-2021 were analyzed. Patients with direct admission and referred condition were compared. Analyses of populations of interest such as patients with severe trauma (ISS > 15) and patients with/without brain trauma were made. The impact of referred patients and their admission status on mortality was evaluated. Results. A total of 10,814 patients were included. The proportion of referred patients was 54.7%. Patients admitted referred vs. with direct admission have differences regarding trauma severity and physiological compromise on admission. The referred patient has a higher risk of mortality (RR: 2.81; 95% CI 2.44-3.22). There is a high proportion of penetrating trauma by gunshot wounds. However, it is the physiological state at admission that impacts mortality. Conclusion. Patients referred from other institutions have a higher mortality risk, being a health inequity that invites the articulation of institutional actors in trauma care. A trauma center should relate to partner institutions to create a trauma system that optimizes care and timeliness

ABSTRACT

Introdução: Lesões por pressão são ocasionadas por aumento local da pressão externa, cursando com isquemia e necrose. Envolvem tratamento multidisciplinar e seu manejo engloba procedimentos conservadores e/ou cirúrgicos. O cirurgião plástico tem atuação primordial no tratamento cirúrgico destas lesões. Métodos: Análise de prontuários de 38 pacientes com lesões por pressão, únicas ou múltiplas, tratados pela Cirurgia Plástica no Hospital do Trabalhador, Curitiba-PR, entre junho de 2012 e março de 2020. Nesse estudo observacional, foram consideradas as características de 45 lesões por pressão juntamente com a técnica cirúrgica empregada e evolução. Resultados: A idade média dos pacientes foi de 35,4 anos, sendo a maioria homens. As lesões localizavam-se majoritariamente nas regiões isquiática, sacral e trocantérica, predominando os graus III e IV. Realizou-se debridamento, seguido por cobertura com retalhos e enxertos de pele, em maioria retalhos de avanço VY e em ilha de glúteo máximo. Alguns pacientes desenvolveram complicações no pós-operatório, principalmente deiscências de retalho e/ou infecção. Conclusão: As lesões por pressão predominaram em pacientes homens adultos jovens, diferentemente da prevalência global (idosos e/ou portadores de doenças crônicas). As maiores incidências ocorreram nas regiões isquiática e sacral, explicadas pela limitação de movimentos incumbida ao perfil de vítimas de grandes traumas. Para tratamento desta lesões, o retalho mais utilizado foi o de glúteo máximo.


Introduction: Pressure injuries are caused by a local increase in external pressure, leading to ischemia and necrosis. They involve multidisciplinary treatment, and their management includes conservative and/or surgical procedures. The plastic surgeon plays a key role in the surgical treatment of these injuries. Methods: Medical records of 38 patients with pressure injuries, single or multiple, treated by Plastic Surgery at Hospital do Trabalhador, Curitiba-PR, Brazil) from June 2012 to March 2020 were analyzed. In this observational study, the characteristics of 45 pressure injuries were considered together with the surgical techniques employed and their outcomes. Results: The average age of the patients were 35.4 years, the majority being men. The lesions were located mainly in the ischiatic and sacral regions, predominating the classification of the III and IV degrees. The lesions were mostly located in the ischial, sacral and trochanteric regions, predominantly grades III and IV. Debridement was performed, followed by coverage with flaps and skin grafts, mostly gluteal VY advancement flaps and gluteal island. Some patients developed complications in the postoperative period, mainly flap dehiscence and/or infection. Conclusion: Pressure injuries predominated in young adult male patients, unlike the global prevalence (elderly and/or patients with chronic diseases). The greatest incidences occurred in the ischial and sacral regions, explained by the limitation of movements charged to the profile of victims of major trauma. The most used flap was the gluteus maximus flap to treat these lesions.

Introducción. El trauma cervical penetrante es una entidad poco frecuente, con tendencia al alza, de considerable morbimortalidad. Es fundamental conocer su manejo por cirujanos en entrenamiento, quienes en su mayoría son comúnmente los proveedores del tratamiento en las áreas de urgencias. El objetivo de este estudio fue demostrar la experiencia de un centro de trauma en México en el tratamiento de trauma cervical penetrante administrado por cirujanos en entrenamiento. Métodos. Estudio retrospectivo de pacientes con diagnóstico de trauma cervical penetrante sometidos a exploración cervical entre los años 2014 y 2019. Se identificaron 110 expedientes, se excluyeron 26 por falta de datos indispensables para la investigación y se analizaron los datos de 84 pacientes. Resultados. El 96,4 % de los pacientes fueron hombres, la mayoría entre 16 y 50 años (83,3 %) y la zona anatómica II fue la más comúnmente lesionada (65,4 %). Las etiologías más frecuentes fueron heridas por arma cortopunzante (67,9 %) y heridas por proyectil de arma de fuego (25 %). El 95 % de los procedimientos fueron realizados por residentes de cuarto y quinto año. La mediana de días de estancia hospitalaria fue de 2 días. La incidencia de complicaciones fue de 9,5 % y la mortalidad de 1,2 %. Conclusiones. Los cirujanos generales en etapa de formación entrenados en centros de trauma tienen la capacidad de tratar de forma óptima el trauma cervical penetrante sin modificación de las tasas de morbimortalidad descritas en la literatura internacional.


Introduction. Penetrating cervical trauma is a rare entity, with an upward trend, of considerable morbidity and mortality. It is essential to acknowledge its management by surgeons in training, who are usually the providers in the emergency areas. The objective of this study was to demonstrate the experience of a trauma center in Mexico in the treatment of penetrating cervical trauma by surgeons in training. Methods. This was a retrospective study of patients diagnosed with penetrating cervical trauma who underwent cervical examination between 2014 and 2019; 110 records were identified, 26 were excluded due to lack of essential data for the analysis, and the data of 84 patients were included. Results. 96.4% of the patients were men, the majority between 16 and 50 years old (83.3%) and the anatomic zone II was the most frequently injured (65.4%). The most common etiologies were stab wounds (67.9%) and gunshot wounds (25%). Ninety five percent of the procedures were performed by fourth- and fifth-year residents. The median number of days of hospital stay was 2 (2-4) days. The incidence of complications was 9.5% and mortality in 1.2%. Conclusions. General surgeons in the trauma training stage can optimally treat penetrating cervical trauma with the same morbidity and mortality rates described in the international literature.

ABSTRACT

Multiple trauma is complex and difficult to treat. The trauma at each site may obscure or delay the manifestation of each other. The severity of injury also exceeds the simple superposition of each injured site. There are numerous definitions of multiple trauma worldwide, with most regarding multiple trauma as major trauma. The definition of multiple trauma in China has evolved several times; however, it is currently still controversial, especially on whether the definition should be based on the nine body regions of the Abbreviated Injury Scale (AIS) or the six body regions of the Injury Severity Score (ISS). The absence of uniform anatomic region criteria in the definition of multiple trauma can lead to discrepancies in multiple trauma patients, causing the clinical application of the definition to be problematic. In this study, the authors elaborate the development and application status of the defination of multiple trauma at home and abroad, discuss the existed problems or controversies and put forward feasible suggestions on the definition of multiple trauma to further normalize the diagnosis and treatment of multiple trauma.

Trauma center is an important link of trauma treatment, which is beneficial for professional integrated treatment of trauma patients and reduction of disability and death rate. The establishment of trauma rescue and treatment system is conducive to improving the regional trauma treatment capacity, rationalizing the layout of medical resources and standardizing the trauma treatment. Making full use of geographic information system (GIS) to analyze the big data such as the number of trauma patients, population, ground traffic situation and geographic information in the region and scientifically plan the number and location of trauma centers in the region is crucial to further optimize the geographic layout of trauma treatment centers, coordinate regional trauma treatment resources and improve the overall treatment capacity and is conducive to optimizing trauma treatment resources, improving regional trauma treatment capacity and reducing the disability and death rate of trauma patients. Based on domestic and foreign literature researches, the authors discuss how to use GIS to optimize the trauma rescue and treatment system in China in order to provide a useful reference for construction of regional trauma rescue and treatment system.

Abstract Introduction: In high-volume trauma centers, especially in developing countries, penetrating cardiac box injuries are frequent. Although many aspects of penetrating chest injuries have been well established, video-assisted thoracoscopy is still finding its place in cardiac box trauma and algorithmic approaches are still lacking. The purpose of this manuscript is to provide a streamlined recommendation for penetrating cardiac box injury in stable patients. Methods: Literature review was carried out using PubMed/MEDLINE and Google Scholar databases to identify articles describing the characteristics and concepts of penetrating cardiac box trauma, including the characteristics of tamponade, cardiac ultrasound, indications and techniques of pericardial windows and, especially, the role of video-assisted thoracoscopy in stable patients. Results: Penetrating cardiac box injuries, whether by stab or gunshot wounds, require rapid surgical consultation. Unstable patients require immediate open surgery, however, determining which stable patients should be taken to thoracoscopic surgery is still controversial. Here, the classification of penetrating cardiac box injury used in Colombia is detailed, as well as the algorithmic approach to these types of trauma. Conclusion: Although open surgery is mandatory in unstable patients with penetrating cardiac box injuries, a more conservative and minimally invasive approach may be undertaken in stable patients. As rapid decision-making is critical in the trauma bay, surgeons working in high-volume trauma centers should expose themselves to thoracoscopy and always consider this possibility in the setting of penetrating cardiac box injuries in stable patients, always in the context of an experienced trauma team.

Abstract Trauma damage control seeks to limit life-threatening bleeding. Sequential diagnostic and therapeutic approaches are the current standard. Hybrid Room have reduced hemostasis time by integrating different specialties and technologies. Hybrid Rooms seek to control bleeding in an operating room equipped with specialized personnel and advanced technology including angiography, tomography, eFAST, radiography, endoscopy, infusers, cell retrievers, REBOA, etc. Trauma Hybrid Service is a concept that describes a vertical work scheme that begins with the activation of Trauma Code when admitting a severely injured patient, initiating a continuous resuscitation process led by the trauma surgeon who guides transfer to imaging, angiography and surgery rooms according to the patient's condition and the need for specific interventions. Hybrid rooms integrate different diagnostic and therapeutic tools in one same room, reducing the attention time and increasing all interventions effectiveness.


Resumen El control de daños en trauma busca limitar el sangrado que amenaza la vida. El enfoque diagnóstico y terapéutico secuenciales son el estándar actual. Las salas híbridas que integran especialidades y tecnologías han reducido el tiempo de hemostasia. Las salas híbridas buscan controlar el sangrado en un quirófano equipado con personal especializado y tecnología de punta como sistemas de angiografía, tomografía, eFAST, radiografía, endoscopia, infusores, recuperadores de células, REBOA, etc. El concepto del servicio hibrido de atención de trauma integra un trabajo vertical que inicia con la activación del código de trauma al admitir un paciente traumatizado grave, iniciando los esfuerzos de reanimación dirigida por el cirujano de trauma que orienta el traslado a salas de imágenes, angiografía y cirugía según la condición del paciente y la necesidad de intervenciones específicas. Las salas hibridas le permite al cirujano de trauma tener todas estas herramientas tanto diagnosticas como terapéuticas al alcance de una sola sala, reduciendo el tiempo de atención y aumentando la efectividad de las intervenciones realizadas.

Abstract The overall incidence of duodenal injuries in severely injured trauma patients is between 0.2 to 0.6% and the overall prevalence in those suffering from abdominal trauma is 3 to 5%. Approximately 80% of these cases are secondary to penetrating trauma, commonly associated with vascular and adjacent organ injuries. Therefore, defining the best surgical treatment algorithm remains controversial. Mild to moderate duodenal trauma is currently managed via primary repair and simple surgical techniques. However, severe injuries have required complex surgical techniques without significant favorable outcomes and a consequential increase in mortality rates. This article aims to delineate the experience in the surgical management of penetrating duodenal injuries via the creation of a practical and effective algorithm that includes basic principles of damage control surgery that sticks to the philosophy of "Less is Better". Surgical management of all penetrating duodenal trauma should always default when possible to primary repair. When confronted with a complex duodenal injury, hemodynamic instability, and/or significant associated injuries, the default should be damage control surgery. Definitive reconstructive surgery should be postponed until the patient has been adequately resuscitated and the diamond of death has been corrected.


Resumen El trauma de duodeno comúnmente se produce por un trauma penetrante que puede asociarse a lesiones vasculares y de órganos adyacentes. En el manejo quirúrgico se recomienda realizar un reparo primario o el empleo de técnicas quirúrgicas simples. Sin embargo, el abordaje de lesiones severas del duodeno es un tema controversial. Anteriormente, se han descrito técnicas como la exclusión pilórica o la pancreatoduodenectomía con resultados no concluyentes. El presente artículo presenta una propuesta del manejo de control de daños del trauma penetrante de duodeno, a través, de un algoritmo de cinco pasos. Este algoritmo plantea una solución para el cirujano cuando no es posible realizar el reparo primario. El control de daños del duodeno y su reconstrucción depende de una toma de decisiones respecto a la porción del duodeno lesionada y el compromiso sobre el complejo pancreatoduodenal. Se recomiendan medidas rápidas para contener el daño y se proponen vías de reconstrucción duodenal diferente a las clásicamente descritas. Igualmente, la probabilidad de complicaciones como fistula duodenales es considerable, por lo que proponemos, que el manejo de este tipo de fistulas de alto gasto se aborde por medio de una laparostomía retroperitoneal (lumbotomía). El abordaje del trauma penetrante de duodeno se puede realizar a través del principio "menos es mejor".

Introducción. El trauma es una de las entidades con mayor morbimortalidad en el mundo. Los equipos especializados en la atención del paciente traumatizado son llamados «equipos de trauma¼. Dichos equipos surgieron de la necesidad de brindar tratamiento oportuno multidisciplinario a individuos con heridas que condicionan gran severidad en la guerra; sin embargo, con el paso del tiempo se trasladaron al ámbito civil, generando un impacto positivo en términos de tiempos de atención, mortalidad y morbilidad. El objetivo de este estudio fue describir el proceso de desarrollo de los equipos de trauma a nivel mundial y la experiencia en nuestra institución en el suroccidente colombiano. Métodos. Se realizó una búsqueda en la base de datos PUBMED, que incluyó revisiones sistemáticas, metaanálisis, revisiones de Cochrane, ensayos clínicos y series de casos. Resultados. Se incluyeron 41 estudios para esta revisión narrativa, y se observó que el tiempo de permanencia en Emergencias, el tiempo de traslado a cirugía, la mortalidad y las complicaciones asociadas al trauma fueron menores cuando se implementan equipos de trauma. Discusión. El diseño de un sistema de atención y valoración horizontal de un paciente con traumatismos severos produce un impacto positivo en términos de tiempos de atención, mortalidad y morbilidad. Se hace necesario establecer los parámetros operativos necesarios en las instituciones de salud de alta y mediana complejidad en nuestro país para implementar dichos equipos de trabajo


Introduction. Trauma is one of the entities with the highest morbidity and mortality in the world. Teams specialized in trauma patient care are called «trauma teams¼. These teams arose from the need to provide timely multidisciplinary treatment to individuals with severe injuries in war; however, with time they moved to the civilian arena, generating a positive impact in terms of care times, mortality and morbidity. The objective of this study was to describe the process of development of trauma teams worldwide and the experience in our institution in southwestern Colombia. Methods. A search of the PUBMED database was carried out, which included systematic reviews, metaanalyses, Cochrane reviews, clinical trials, and case series.Results. Forty-one studies were included for this narrative review, and it was observed that the length of stay in the ER, the time of transfer to surgery, mortality and complications associated with trauma were lower when trauma teams are implemented. Discussion. The design of a horizontal care and assessment system for a patient with severe trauma produces a positive impact in terms of care times, mortality and morbidity. It is necessary to establish operational parameters in high and medium complexity health institutions in our country to implement such work teams

PURPOSE@#This study aimed at exploring the application of trauma time axis management in the treatment of severe trauma patients by using the Medicalsystem trauma system.@*METHODS@#We performed a retrospective cohort study involving patients with severe trauma. Patients who were admitted before the application of the Medicalsystem trauma system were divided into before system group; patients who were admitted after the application of the system were divided into after system group. Comparison was made between the two groups. For normally distributed data, means were reported along with standard deviation, and comparisons were made using the independent samples t test. Categorical data were compared using the Chi-square test. The Mann-Whitney U test was used to compare nonparametric variables.@*RESULTS@#There were 528 patients admitted to the study during the study period. There was no significant statistical difference in the time from the start of trauma team to arrive at the resuscitation room between the two groups. The time from arrival at hospital to endotracheal intubation, to ventilator therapy, to blood transfusion, to completion of CT scan, to completion of closed thoracic drainage, to the start of operation, as well as the length of stay in resuscitation room and hospital were significantly lower after the application of the Medicalsystem trauma system. The mortality was decreased by 8.6% in the after system group compared with that in the before system group, but there was no statistical difference.@*CONCLUSION@#The Medicalsystem trauma system can optimize diagnosis and treatment process for trauma patients, and accordingly improve the treatment efficiency and shorten the treatment time. Therefore, the Medicalsystem trauma system deserves further popularization and promotion.

Severe trauma is almost caused by a high energy damage, with a high incidence rate of multiple injuries. Reasonable assessment and resuscitation in hospital at early stage is key to successful care. Trauma resuscitation bay(TRB)is the primary area for early assessment and resuscitation in hospital, which can ensure the rapid rescue to the greatest extent. At present, the early treatment for patients with severe trauma is mainly conducted in the emergency room, which presents many disadvantages that may lead to a decline in the quality of early treatment of trauma. For example, in the limited space, a resucue team is unable to carry out, and bedside radiographic examinations, life-saving operations and rescue for internal emergency patients are affected. With the development of trauma center construction, TRB specifically for early assessment, resuscitation and emergency management of severe trauma comes into being. According to the characters of China's trauma rescue system, the authors illustrate the characteristics, advantages, functions and operational requirements of TRB, the components of TRB at different levels of trauma centers, and the status of TRB construction in China, so as to provide a reference for the construction of TRB at various levels.

Abstract Hemorrhagic shock and its complications are a major cause of death among trauma patients. The management of hemorrhagic shock using a damage control resuscitation strategy has been shown to decrease mortality and improve patient outcomes. One of the components of damage control resuscitation is hemostatic resuscitation, which involves the replacement of lost blood volume with components such as packed red blood cells, fresh frozen plasma, cryoprecipitate, and platelets in a 1:1:1:1 ratio. However, this is a strategy that is not applicable in many parts of Latin America and other low-and-middle-income countries throughout the world, where there is a lack of well-equipped blood banks and an insufficient availability of blood products. To overcome these barriers, we propose the use of cold fresh whole blood for hemostatic resuscitation in exsanguinating patients. Over 6 years of experience in Ecuador has shown that resuscitation with cold fresh whole blood has similar outcomes and a similar safety profile compared to resuscitation with hemocomponents. Whole blood confers many advantages over component therapy including, but not limited to the transfusion of blood with a physiologic ratio of components, ease of transport and transfusion, less volume of anticoagulants and additives transfused to the patient, and exposure to fewer donors. Whole blood is a tool with reemerging potential that can be implemented in civilian trauma centers with optimal results and less technical demand.


Resumen El choque hemorrágico y sus complicaciones son la principal causa de muerte en los pacientes con trauma. La resucitación en control de daños ha demostrado una disminución en la mortalidad y mejoría en el manejo del paciente. La resucitación hemostática consiste en la recuperación del volumen con hemoderivados como glóbulos rojos, plasma, crioprecipitado y plaquetas, en proporciones de 1:1:1:1. Sin embargo, esta demanda de hemo componentes podría no aplicarse para toda Latinoamérica u otros países de medianos y bajos ingresos. Las principales barreras para la implementación de esta estrategia serían la escasa disponibilidad de bancos de sangre y de hemoderivados insuficientes para contar con un protocolo de transfusión masiva. Una propuesta para superar estas barreras es el uso de sangre total fresca fría para la resucitación hemostática de los pacientes exsanguinados. Ecuador ha sido pionero en la implementación de esta estrategia con una experiencia ya de seis años, en que han demostrado que la sangre total tiene ventajas sobre la terapia de hemo componentes incluyendo, pero no limitando, la trasfusión de sangre con una razón fisiológica de componentes, fácil transporte y transfusión, menor volumen de anticoagulantes y aditivos trasfundidos al paciente, y menor exposición a donantes. La sangre total es una herramienta con un potencial reemergente que puede ser implementado en centros de trauma civil con óptimos resultados y menor demanda técnica.

Abstract Noncompressible torso hemorrhage is one of the leading causes of preventable death worldwide. An efficient and appropriate evaluation of the trauma patient with ongoing hemorrhage is essential to avoid the development of the lethal diamond (hypothermia, coagulopathy, hypocalcemia, and acidosis). Currently, the initial management strategies include permissive hypotension, hemostatic resuscitation, and damage control surgery. However, recent advances in technology have opened the doors to a wide variety of endovascular techniques that achieve these goals with minimal morbidity and limited access. An example of such advances has been the introduction of the Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA), which has received great interest among trauma surgeons around the world due to its potential and versatility in areas such as trauma, gynecology & obstetrics and gastroenterology. This article aims to describe the experience earned in the use of REBOA in noncompressible torso hemorrhage patients. Our results show that REBOA can be used as a new component in the damage control resuscitation of the severely injured trauma patient. To this end, we propose two new deployment algorithms for hemodynamically unstable noncompressible torso hemorrhage patients: one for blunt and another for penetrating trauma. We acknowledge that REBOA has its limitations, which include a steep learning curve, its inherent cost and availability. Although to reach the best outcomes with this new technology, it must be used in the right way, by the right surgeon with the right training and to the right patient.


Resumen La hemorragia no compresible del torso es una de las principales causas de muerte prevenibles alrededor del mundo. Una evaluación eficiente y apropiada del paciente traumatizado con hemorragia activa es la esencia para evitar el desarrollo del rombo de la muerte (hipotermia, coagulopatía, hipocalcemia y acidosis). Actualmente, las estrategias de manejo inicial incluyen hipotensión permisiva, resucitación hemostática y cirugía de control de daños. Sin embargo, los recientes avances tecnológicos han abierto las puertas a una amplia variedad de técnicas endovasculares que logran esos objetivos con una morbilidad mínima y un acceso limitado. Un ejemplo de estos avances ha sido la introducción del balón de resucitación de oclusión aortica; REBOA ( Resuscitative Endovascular Balloon Occlusion of the Aorta , por sus sigla en inglés ), el cual, ha tenido gran provecho entre los cirujanos de trauma alrededor del mundo debido a su potencial y versatilidad en áreas como trauma, ginecología y obstetricia, y gastroenterología. El objetivo de este artículo es describir la experiencia lograda en el uso del REBOA en pacientes con hemorragia no compresible del torso. Nuestros resultados muestran que el REBOA puede usarse como un nuevo actor en la resucitación de control de daños del paciente con trauma severo, para este fin, nosotros proponemos dos nuevos algoritmos para el manejo de pacientes hemodinámicamente inestables: uno para trauma cerrado y otro para trauma penetrante. Se reconoce que el REBOA tiene sus limitaciones, las cuales incluye un periodo de aprendizaje, su costo inherente y la disponibilidad. A pesar de esto, para lograr los mejores resultados con esta nueva tecnología, el REBOA debe ser usado en el momento correcto, por el cirujano correcto con el entrenamiento y el paciente correcto.
